With 500 pages total and 6 perforated, easy-removal slips per page, there is plenty of passes to go around for the classroom or school main office. Each tardy slip has lines for jotting down student’s name, date, time he/she arrived, and reason for tardiness; bottom half of slip features area to fill in “excused” or “unexcused” by the teacher. All original white sheets are perforated (for easy tear-off removal) while the duplicate copy stays in the copy for adult authority to keep for tracking each individual student’s number of late arrivals. Page size is 8-1/2 x 11 inches; each tardy slip measures 3-3/4 x 3-5/8 inches. Record book has 50 pages with 6 tardy slips each (for a total of 300 slips).
